Fall/Winter 2008-2009 Registration Packet (pdf)
(includes all of the forms below as well as other important information)

All the following forms need to be filled out completely and turned in when registering:

Family Info/Group Designation Form (pdf)
Please review the Practice Times and Fees Sheet for squad charges.   
Everyone must mark House Account or provide a credit card # for all charges, including escrow. 

Swimmer Info Form (pdf)
Please mark Fall Season, or Winter Season, or Fall & Winter Season.
(Your charge will be based on the squad your swimmer is placed on)

Each swimmer will be charged a $15 Administration fee for Fall, and a $35 Administration fee for Winter.  These Administration fees will be used to cover general swim team expenses, and swimmer incentive programs.  Checks should be made out separately to WSC Stingrays.

ISI Form (pdf) OR ISI Transfer Form (pdf)
Attach a $60 (or $10 transfer fee) check payable to
I.S.I. Every swimmer must be ISI registered, regardless if they are
swimming in any meets or not.

Explanation of Escrow Account
The first meet you are entered in, if you escrow account is less than your
meet fees, your house account, or credit card (whichever was marked on your Family
Info Form) will be charged $50. Any subsequent meets you are entered in, in which
the amount in your escrow account is less than $25, your account will be charged
$25. At the end of each swim season, any escrow accounts with a negative balance
will be charged the amount to zero out their escrow account.
